Lessons Learned from Accidents – 2000

This report contains the proceedings of the meeting of environmental inspectors on industrial accidents held in Lyon on 6 and 7 June 2000.
In total 16 accidents, most of them in France, were reported. Each report has a description of the incident, the origin, causes and circumstances of the accident, actions taken and conclusions including lessons learnt.
